Click here to make changes to your Family ID registration: HHS FAMILY ID
Do not send your child's physical exam to the nurses for Athletics.
New physicals must be uploaded as part of the registration process.
Do not send Athletics a hard copy of your physical.
Physical exams are good for 13 months. Once uploaded, Family ID will automatically notify you (and HHS Athletics) before the 13 months are up, letting us know that we need to collect a new physical form. We are hopeful that this will be a benefit to families as well.
Uploading a NEW physical to a current registration:
You should be able to go into your registration and click on "Add or Update Information" for any completed registration, this will let you back in to upload their physicals whether the program is closed or not.
